Ace & Tate will check eligibility when you provide your PPS number.
Free PRSI eye tests and free glasses are benefits offered to eligible individuals under the Pay-Related Social Insurance (PRSI) scheme. This program aims to support eye health by providing access to essential eye examinations and prescription eyewear at no cost.
Your PRSI number can be located on your payslip, tax documents, public services cards, in your Employment Detail Summary and your PRSI record. Employment Detail Summaries can be accessed online through Revenue’s myAccount service.
A copy of your complete PRSI record can be found online at MyWelfare.ie
Claims are made by Ace & Tate and we will check your entitlement in advance of treatment and claim payment afterwards. To do this we will require some information to identify you, such as date of birth and PRSI number.
Prior to treatment, you will need to sign a consent form agreeing to share this information with us and the DSP, this will enable payment of your claim. If you are claiming as a dependent under the PRSI record of your spouse, civil partner or cohabitant, you will both need to sign the consent form.
Unfortunately, this is not possible, the invoice will have the purchase date on it.
Your insurance provider will not consider it done in 2024, because the invoice will have the purchase date on it.
